Charcot foot is a serious complication of diabetic neuropathy where bones in the foot weaken, fracture, and collapse. Early diagnosis and offloading prevent deformity and amputation.

Charcot neuroarthropathy most often affects patients with diabetes and neuropathy. The foot becomes red, warm, and swollen — often mistaken for infection or a sprain — while bones silently fracture and the arch collapses.

Early diagnosis with X-ray (sometimes MRI), strict offloading with a total contact cast or CROW boot, and long-term custom bracing prevent the rocker-bottom deformity that leads to ulcers and amputation. Severe deformities may require surgical reconstruction.

Frequently asked questions

How long do I have to wear a cast for Charcot foot?

Total contact casting is typically used for 3–6 months until the foot is no longer warm and swollen, then transitioned to a CROW boot or custom diabetic shoes for long-term protection.

Can Charcot foot be reversed?

Active inflammation can be stopped with strict offloading, but bone changes that have already occurred are permanent. The goal is to prevent further collapse and protect the foot from ulcers.
