Office Information: About Your Visit
Your first visit to Foot Health Centers establishes a vital foundation for our relationship with you. At this time we will make sure to obtain important background information, like your medical history and give you time to get to know our doctors.
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
What To Bring
Please be prepared by bringing your insurance cards, drivers license, a list of all current medications and surgical history. You can even save yourself some time by printing and filling our Office forms. These forms can be brought with you on the day of your appointment.
The physicians at Foot Health Center are contracted with most major insurance companies but it is our suggestion that you contact our office to verify any co-pay or deductible requirements prior to your visit.
Fees & Payments
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept personal checks, cash, Visa and Mastercards. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient’s responsibility. We will help in any way we can to assist you in handling claims.
Prescriptions & Renewals
Please request all prescriptions at the time of your visit when we have your complete medical history in front of us. Requests outside of this time will only be filled for extreme or post surgery circumstances.
